Enrollment Confirmation 2024-2025

Please fill out the information below to confirm receipt of your child's 2024-2025 enrollment agreement.  By submitting this form, you agree to the schedule and corresponding monthly tuition payments that were outlined on the paper.

The first payment, which is an advance deposit for the month of June 2025,

will be invoiced on ProCare with an autopay date of 2/15/24.
